Changes to the substance/meaning/intent/message of an entry are always noted. I sometimes change angry/vent entries to private instead of editing, because I don't feel they need to be out there, muddying up people's lists, and because I know that I won't feel the same or I'll be coping different in a brief period of time.
I don't really go back and change the substance, but I will frequently (nearly always) edit my last entry for typos, or add something, or whatever. If I add something totally new, I'll usually mark it as such.
Occasionally I'll refilter something or make it private if I was being a total idiot, but yeah, I don't have any problem doing that.
I write so little that I have not had cause to edit. But I like the pattern among some of my friends of putting major comment-responses or plain updates in edits, to make the reading order more sensible. Of course, there are all the issues to which you allude, because LJ lacks a good content subscription model.
Well, usually I just leave it as is, or I'll make a note "edited to add". But sometimes I go back to fix grammar or spelling, and I don't make a note of that.

i always edit for typos/grammar and i don't indicate that. right after posting, i'll read back through it when the entry pops up on my flist, and if there are no comments i might make certain kinds of changes (like clarifications) without noting that i did so. if i later add/change something of substance, i'll note it. otherwise comments might not make sense.
A friend of mine once made a game out of that. The not-so-open-ended "question of the week" was posted every week, and after a couple of days and a bunch of responses, the question was changed. Good times.
